The client authorizes Precious Paws Spa and Snoozes staff to perform the agreed-upon grooming services on the pet, as discussed during the consultation. The client understands that:
While every effort is made to ensure the pet's safety and comfort during grooming, accidents can occur. Possible risks include, but are not limited to, nicks from clippers, scissors, or nail trimmers, skin irritation, razor burn, and stress for young, geriatric, timid, or temperamental pets.
If the pet displays aggression or is unable to be safely groomed, Precious Paws Spa and Snoozes reserves the right to refuse or stop grooming services at any point. In such cases, the client will be charged for the services rendered up to that point.
Matted Dog Policy: If the pet's coat is matted, de-matting may cause discomfort or pain and may require a shorter haircut than initially desired. If matting is severe, it may be necessary to shave the coat down for the safety and well-being of the pet. Precious Paws Spa and Snoozes is released from responsibility for any injury, skin irritation, or after-effects resulting from grooming a matted pet.
In the event of a medical emergency during grooming, Precious Paws Spa and Snoozes will attempt to contact the client or their emergency contact first. If the client cannot be reached, Precious Paws Spa and Snoozes is authorized to seek necessary veterinary care for the pet at the client's expense.
Precious Paws Spa and Snoozes may take photographs and/or videos of the pet for education, advertising, and social media.
The client is responsible for informing Precious Paws Spa and Snoozes of any and all medical or behavioral conditions, allergies, or concerns regarding their pet.
All fees for grooming services are due and payable at the time services are rendered.
The client will provide at least 24 hours notice for any cancellations or rescheduling requests to avoid a potential cancellation fee.

Medical treatment: If your pet develops any medical condition during their stay we reserve the right to treat as necessary to prevent the spread of illness. In a life-threatening emergency we will make every attempt to contact you or an authorized agent listed on this form; if we cannot reach you we will proceed as the doctor deems necessary. All treatments are your financial responsibility.
General rules:
If your pet is picked up by someone other than you, arrangements (including payment) must be made at drop-off, and that person must show ID.
We are not staffed 24 hours a day; pets are unattended during evening hours.
To extend a stay, contact us 24 hours before the expected discharge date. A pet not picked up within 10 days of the expected discharge is considered abandoned; you remain responsible for all charges.
All animals must be free of fleas & ticks when admitted, or will be treated on arrival at the owner's expense.
Boarding animals are admitted and discharged only during regular posted business hours.
If your pet shows aggression to staff, we reserve the right to end the stay immediately.
All precautions are taken to prevent injury or escape; staff are not held accountable for problems that develop where reasonable care has been taken.
Annual vaccinations and fecal exams are required for boarding.
We may take photos or videos of your pet for education, advertising, and social media.
The most we can spend on emergency vet care before we must reach you first. Leave blank for no limit.
